Our physical, occupational and speech/language therapists work with residents nationwide confronting a variety of conditions, from stroke to dysphasia to Parkinson’s disease, as well as conditions resulting from elective surgeries, multiple trauma or recovery from amputations. The rehabilitation team creates an individualized treatment program developed with the interdisciplinary team of nursing, social services, activities and dietary in consultation with your physicians. The team members’ objective is to play an integral role in helping you regain your health and reach your maximum functional ability.
Occupational Therapy Outcomes
Occupational therapy is provided to residents with the goal of improving their ability to complete activities of daily living. The focus is on activities such as bathing/showering and grooming, household tasks such as cooking and cleaning and community activities such as shopping and transportation. Transfer training and standing balance while completing household tasks may also be focused on. Upon evaluation, a resident is assigned an FIM score, or a Functional Score, for each activity they will be completing while working with Occupational Therapy. Goals are developed based on a resident’s needs at home and in the community and a fluid discharge plan is set up. Once a resident has achieved his goals necessary to return home safely, FIM scores are assigned to convey a resident’s current status at discharge. As shown in the graph below, our residents consistently improve in all areas of assessment, achieving a level requiring only supervision to modified independence.
Physical Therapy Outcomes

Stairs and car transfers may also be focused on in PT depending on patient need. If a patient is unable to ambulate, a PT may train in wheelchair management to ensure independent mobility for the patient. Upon evaluation, a resident is assigned a FIM score, or a Functional Score, for each activity they will be completing. Goals are developed based on a resident’s needs at home and in the community and a fluid discharge plan is setup.
